No, he really didn't call Sarah Palin a pig. The phrase "put lipstick on a pig" is an old phrase. It's used to talk about how you can't take something that's awful (aka the pig,) attempt to dress it up (aka the "lipstick,") and then try to call it anything other than a pig.

He's talking about the bush administration's policies and how they've turned the country to shit. You don't get to dress it up by going "we're going to change, I promise!" Hence why he reiterated the comment with the fish analogy. It would not be surprising in the least if the Obama comments in that video were made before the Sarah Palin joke.
